You also need a care team that is in sync with your physician to ensure the treatment you receive is designed to help you regain the mobility, strength, independence, and overall wellness that you’re seeking. 

Horizons offers personalized recovery and therapy programs through a specialty team approach to provide the most personalized care possible. But your recovery goes beyond care, which is why we also have a dedicated guest services team that is dedicated to making your stay as comfortable as possible and handling any requests that you may have. Our community also features many amenities and social activities that are available to you throughout your stay, which are a real boost to mental and emotional health while going through the recovery process.

Residents of Horizons’ state-licensed long-term nursing community neighborhoods receive highly skilled and personalized care and various personal services, all within the welcoming beauty and comfort of our Eckert, CO campus. We utilize specially trained Optimal Care teams of nursing assistants, licensed nurses, rehabilitation therapists, and other professionals that collaborate directly with each resident and their physician to develop individualized care plans that emphasize both health and independence. 

We’re committed to person-centered care and the overall quality of life of our residents, not just their physical health. Every person who lives as part of our long-term nursing community has access to life enrichment programming, including spiritual services, a monthly schedule of social activities, participation in local area events, and various educational opportunities. 

Our outpatient therapy services are provided by an expert therapist and focused on improving mobility and strength while reducing pain through exercise and manual therapy. Outpatient therapy services are available Monday through Friday in our on-campus therapy gym. 

Respite care is a short-term, supportive option for caregivers and their loved ones when caregivers need temporary assistance. Guests in our respite care program receive the same attentive, compassionate care and services as our long-term residents, including individualized care plans, access to our conveniences, calendar of activities, amenities, and the welcoming and beautiful environment of Eckert, Colorado. 

In addition to providing extra support for caregivers, our respite care services can also be beneficial for those recovering from illness or hospitalization.
